Application forms to download and fill in to apply for our IV Insertion Certification Validation Program.

How to apply:

1. Place an order for the forms, and in the process create an account with us by clicking the button labeled "Add to Cart", next the checkout button, and follow the page instructions to make an account with us and complete your order  (There is no charge to download the forms).
2. Click on the download link on the last page or within your email to download the forms
3. Fill in the application forms as directed within the package, and complete one of our IV Insertion Classes as part of meeting program requirements (see qualifiying classes listed below)
4. Before submitting an application and fee; review the program requirements for successful completion that are found within the application package to ensure that you meet the program guidelines.  The forms are in pdf format.

To view the download files you will need Adobe's PDF Reader program.  If you do not have Adobe Reader, you can download a free program at http://www.adobe.com .

Completion of one of our Short IV Insertion classes is required for completion of the IV Insertion Certification Validation program.  Choose from one of our IV insertion class options: The Short IV Insertion In-Person classes held in Connecticut, or our Short IV Insertion Class online or via DVD ROM programs.

What to expect after completing your order/request for the Application Forms:

Immediate download file access: You will be directed to a download page to download one pdf file that contains copies of the program application forms.

Emails sent by our automated system: You will receive a few emails- 1st to verify that we received your order, and next you will receive a confirmation email with a copy of an invoice receipt.  The confirmation email will contain a link to the file download that are also included on the download page immediately following completion of your order.  There is no charge to download the application forms.  You will receive the email confirmation in most cases within a few minutes.
